Life Health and Wellness Center was created to serve the homeless men, women and families. Our providers also serve Veterans and victims of Human Trafficking. Our clinic does not discriminate against race, color, sex, religion, sexual orientation, or ability to pay. We are a group of health care providers that provide family medicine to our patients. Many of our patients do not have health care but they do have chronic illnesses that have not been treated in years. We have contracts with the local universities and provide clinical education to students for Primary Care clinical rotations. The student are taught use evidence based methods to assess, diagnosis and treat patients and also educate patients about their current condition. We are also developing ways to sign patients up with insurance so they can become compliant with medications and therapies ordered to maintain positive health outcomes.

AshaUSA's m​ission is to provide culturally specific programs to the South Asian community t​​o foster health and harmony in their lives. Formed in July, 2014 in Minnesota, AshaUSA (Asha means hope) is a non-profit organization focused on engaging and empowering South Asian women, men, and children through culturally specific programs and services. Over 3.4 million South Asians (originally from India, Pakistan, Bhutan, Nepal, Bangladesh and Sri Lanka) live in the United States out of which about 44,000 live in Minnesota (2010 census). 75% of South Asian Minnesotans are first generation immigrants. The South Asian population in Minnesota has more than doubled since 2000. With the increase in population, challenges associated with acculturation in terms of health, youth development, and family relationships have become more distinct. There is a need for culturally specific programs that proactively address acculturation challenges for the South Asian community in Minnesota. ​

Since 1961, PPT has been providing information and counselling to the City of Toronto. From the latest in contraception choices to anonymous HIV testing, our resources and programs have kept pace with the changing needs of the communities we serve. Today, we are a fully accredited community health centre that provides primary and sexual health care services to youth ages 13 to 29 years, as well as health promotion programming, education, training and research to improve the health and well-being of Toronto’s diverse communities. Our services are offered within a pro-choice, inclusive, youth-friendly, non-judgmental environment. Our programs include: A Community Health Centre for youth ages 13-29, a satellite program for women throughout their reproductive years at the Bay Centre for Birth Control, Teen Programming, including TeenHealthSource.com, Teens Educating and Confronting Homophobia (T.E.A.C.H.), Health Promotion, and volunteer opportunities. We serve the Region of Peel (Missisauga, Brampton and Caldeon). Through our community education and outreach programs, we pro-actively communicate with the general public and at-risk populations on HIV prevention, healthy sexuality and harm reduction. Our Health Promotion Team has specialized outreach programs, which are targeted to Men who have sex with Men (MSM), the African Diaspora, and Injection Drug users and women who are are-risk for HIV infection. Our PHA support programs provide individuals with a range of support services, counselling, health promotion and capacity building, and referrals to improve quality of life. Our Volunteer program develops and supports a dynamic, well-trained volunteer team which reflects our community and serves the needs of our client base. All our programs collaborate with a broad base of networks, organizations and community members in planning, service delivery and evaluation.

The SHARP Foundation is a Calgary based, non-profit, charitable organization that provides a spectrum of integrated (medical, physical, psychosocial and spiritual aspects) care to our community's most vulnerable persons living with HIV/AIDS. We offer hope, dignity and compassion in a safe, non-judgemental environment. SHARP currently operates 4 residential care facilities: • Project Kathleen - 24/7 supported care for women with complex health/psychosocial profiles • Beswick House - a 24-hour supportive living environment for 10 people living with HIV / AIDS, including palliative care support. • Scott House - a program to address the needs of clients who require support to successfully establish themselves into independent living situations. • Project 2011 - Provides longer term housing and supports for individuals who have demonstrated an ability to live more independently in a group home setting. We are a collaborative, inner-city Winnipeg, 9-site community outreach organization especially reaching marginalized women/families expecting a baby or parenting a baby to 1 year. Health & nutrition from the start are encouraged by welcoming, interactive, practical educational groups and one-on-one support. Healthy low-cost snacks, ready-to-cook Meal Bags for $1.50, childminding at groups, milk coupons (as financially needed), language interpreting at 1 location, & access to friendly nurse, dietitian or outreach workers are offered. Results include: Prevention of low birth weight (LBW) babies, Prevention of FASD (Fetal Alcohol Spectrum Disorder) Increased breastfeeding and increased protective health factors for babies Increased nutritional and health knowledge Identifying failure to thrive babies Improved parent confidence, knowledge, skills Supported by the Canada Prenatal Nutrition Program and Healthy Child Manitoba as well as by donations & sales of educational posters.

Since 1964, West Ridge Academy (formerly known as the Utah Boys Ranch) has been helping struggling teens and bringing hope to young men, young women and their families. From the earliest days, the organization recognized the positive effect its program had on young men who were struggling academically, emotionally and socially. Today, the academy has served more than 25,000 students, has expanded to include separate and distinct campuses for boys and girls, and continues to bring hope and healing to families across the world.     The purpose of West Ridge Academy is simple: to send a child home with a change of heart. We are consistently able to do this by applying our foundational principles of Family, Spirituality, Integrity, Work, Service and Gratitude. With each student, a Master Treatment Plan is individually developed and implemented, created specifically for each student’s needs. Every Treatment Plan includes guidance and personal involvement from four teams including Therapists, Home Staff, Teachers and Spiritual Advisors.

Sexual Health Centre Saskatoon (SHCS) provides a non-judgmental, sex -positive, and supportive atmosphere to help you make decisions and take action that is right for you. SHCS provides factual, up-to-date and unbiased information regarding all methods of contraception which can help you in planning and/or avoiding pregnancy. We provide information and counseling about HIV/AIDS and other STI’s. Weekly we offer a drop-in youth focused birth control and sexual health clinic. A nurse is on hand to offer physical exams, birth control prescription and STI testing. SCHS offers most types of birth control in the office at discount prices as well as free condoms. SCHS offers the opportunity for you to explore all of your options when faced with an unintended pregnancy. We believe that women have the right to non-judgmental support and factual information about all options—parenting, adoption, and abortion in order to make confident, well informed decision

Hudson Pride Connections Center (HPCC) is a home and voice for the diverse LGBTQ community and our allies that advocates for our physical, mental, social and political well-being. We create safe and vibrant spaces to gather and celebrate our lives. HPCC is a 501(c)(3) non-profit, non-governmental, community organization situated in the Journal Square neighborhood in Jersey City, one of the most vibrant and diverse cities in this country and home to the largest LGBTQ community in New Jersey. Hudson Pride was established in 1993 to serve as an advocate for both the LGBTQ and HIV/AIDS communities in Hudson County and more than 20 years later, we are still delivering a wide array of services, programs and events to meet their on-going needs. Our programs and services include LGBTQ support groups for youth, young adults, middle adults and seniors, specialized groups for HIV+ gay men, transgender individuals and women of color, community training and education focused on helping provider agencies to deliver LGBT and HIV competent services, LGBTQ health education, HIV outreach services, social service linkages and advocacy.

BAMSI is one of the largest minority, non-profit, human service organizations in Massachusetts. BAMSI's workforce of nearly 2,000, provides the highest quality of care, services and resources designed to help individuals achieve a community based, quality lifestyle. BAMSI's geographic footprint of services spans from Worcester to Plymouth and Tewksbury to Attleboro serving nearly 30,000 individuals per year at nearly 130 program locations.
