Our mission is to create self-sustainable programs that empower women and their families. We strive to improve their health, education, and economic status in the communities where they live. These sustainable strategies are culturally, environmentally and practically sound. While we aim to be pragmatic, results-oriented organization that partners with other like minded organizations, we monitor outcomes. Our goal is to be scientifically sound and operationally transparent.

Life Health and Wellness Center was created to serve the homeless men, women and families. Our providers also serve Veterans and victims of Human Trafficking. Our clinic does not discriminate against race, color, sex, religion, sexual orientation, or ability to pay. We are a group of health care providers that provide family medicine to our patients. Many of our patients do not have health care but they do have chronic illnesses that have not been treated in years. We have contracts with the local universities and provide clinical education to students for Primary Care clinical rotations. The student are taught use evidence based methods to assess, diagnosis and treat patients and also educate patients about their current condition. We are also developing ways to sign patients up with insurance so they can become compliant with medications and therapies ordered to maintain positive health outcomes.

We provide information, education, support and advocacy for people with hearing loss in Contra Costa, Eastern Alameda and Southern Solano counties. Hearing loss affects all parts of your life; communicating with loved ones, colleagues and friends; going to the grocery store, pharmacy, doctors appointment; and entertainment and sporting events. We help each other learn how to live functional lives with a difficult disability.

LiveWell Colorado is a nonprofit dedicated to fighting obesity by increasing access to healthy food and physical activity. In addition to educating and inspiring people to make healthy choices, LiveWell Colorado focuses on policy and environmental changes that remove barriers to healthy living opportunities. Working in partnership with obesity prevention initiatives across the state, LiveWell aims to provide every Coloradan with access to healthy food and opportunities for physical activity.

At Valley Center for the Blind we join together in a spirit of dedication to assist blind and visually impaired persons in experiencing a more independent and opportunity filled life. These efforts are achieved through specialized methods of training and education provided by our professional staff. We believe that every individual, regardless of disability, can accomplish their desired goals provided the proper instruction and support. With over 40 years of experience, Valley Center for the Blind believes that every individual should have the opportunity to live independently and we teach adaptive techniques to make this a possibility for the visually impaired. Our primary goal is to support each and every client that walks through our doors. Whether it be educational support from our classes, emotional assistance from our counselors, or a new camaraderie found in one of our activities. We aspire to serve anyone dealing wtih vision loss and vision impairment by helping them reestablish independence, build confidence, and find new ways to enjoy life!

The mission of Brain Injury Services, Inc. is to improve the lives of people who are affected by brain injury through community-based services, education and advocacy. The purpose of adapt clubhouse is to encourage an atmosphere of many possibilities and provide an environment where members work together supporting and encouraging each other. Members gain vocation skills, increase their level of independence, and social skills. Staff and members work together in a cooperative work ordered day experience.

Maji Safi Group’s mission is to promote health and disease prevention in underserved and impoverished areas through holistic community empowerment and by working predominantly with local women and youth. MSG was founded in 2013 with the goal of preventing waterborne diseases through WASH education by putting local community members at the center of driving changes in public health. Our mission revolves around a model of participatory development that involves community members in all aspects of our work.

"To provide a centralized statewide agency with auxiliary support services for deaf, hard-of-hearing, deaf-blind, late-deafened persons, including persons with hearing losses and those hearing persons with loss of voice, regardless of race, age, and gender such that they can be provided opportunities to succeed in areas of employment; education; housing; economics; and independence; and also be provided legal and fair judicial representation which are seldom provided. Such opportunities and legal rights are currently not obtainable or are being denied."

Brave Space Alliance is the first Black-led, trans-led LGBTQ+ Center located on the South Side of Chicago, dedicated to creating and providing affirming, culturally competent, for-us by-us resources, programming, and services for LGBTQ+ individuals on the South and West sides of the city. We strive to empower, embolden, and educate each other through mutual aid, knowledge-sharing, and the creation of community-sourced resources as we build toward the liberation of all oppressed peoples.

Our mission is to mobilize a community for patient support and advocacy, as well as drive and support impactful research that translates into advances for patients with migraine and other disabling diseases that cause severe head pain. Our core beliefs guide everything we do, from the research we promote to our efforts to build a supportive, welcoming community for all people living with migraine. Advocacy and education is the only way we will increase awareness and recognition of migraine for the disabling disease it truly is.

The Indiana Recovery Alliance operates under the philosophy of harm reduction to educate the community about health risks and to promote the health and dignity of the individuals and communities impacted by drug use. We respectfully collaborate with people to assist in any positive change, as a person defines it for themselves, beginning where the person is at with no biases or condemnations for the person's chosen lifestyle. Our efforts advance policies, practices and programs that address the adverse effects of drug use including overdose, HIV, hepatitis C, addiction and incarceration.

Hudson Pride Connections Center (HPCC) is a home and voice for the diverse LGBTQ community and our allies that advocates for our physical, mental, social and political well-being. We create safe and vibrant spaces to gather and celebrate our lives. HPCC is a 501(c)(3) non-profit, non-governmental, community organization situated in the Journal Square neighborhood in Jersey City, one of the most vibrant and diverse cities in this country and home to the largest LGBTQ community in New Jersey. Hudson Pride was established in 1993 to serve as an advocate for both the LGBTQ and HIV/AIDS communities in Hudson County and more than 20 years later, we are still delivering a wide array of services, programs and events to meet their on-going needs. Our programs and services include LGBTQ support groups for youth, young adults, middle adults and seniors, specialized groups for HIV+ gay men, transgender individuals and women of color, community training and education focused on helping provider agencies to deliver LGBT and HIV competent services, LGBTQ health education, HIV outreach services, social service linkages and advocacy.
