NashvilleCARES’ MISSION is to end the HIV/AIDS epidemic in Middle Tennessee. We work toachieve this through education, advocacy and support for those at risk for orliving with HIV. Our VISION is acommunity where HIV infections are rare, and when they do occur, everyone withHIV/AIDS has access to the care, treatment and support to achieve optimalhealth and self-sufficiency without stigma or discrimination.

1. CHAP educates the public about HIV disease, its impact upon individuals and the community, and current treatments;2. CHAP coordinates the efforts of existing public or private resources for persons living with HIV/AIDS;3. CHAP works to discover unmet needs of persons with HIV/AIDS and their families;4. CHAP stimulates and supports appropriate community responses to meet those needs.

Since our founding, we have been committed to fighting the HIV epidemic until this crisis is over. Advances in treatment and new prevention strategies have now made “Getting to Zero” – zero stigma, zero new infections, and zero deaths – an achievable goal! The focus of all of our programs is to help achieve this goal. Our From All Walks of Life Fund supports the most vulnerable people living with HIV through financial assistance grants during times of crisis. Our annual back-to-school and holiday toy drives support HIV infected/affected youth. Our public awareness and education efforts are designed to combat the stigmas associated with HIV, and to raise awareness and educate the community about the newest prevention strategies, including PrEP (pre-exposure prophylactics), PEP (post exposure prophylactics), TasP (treatment as prevention) and the advances in the care and treatment of HIV.

Identity works with Latino and other historically underserved youth and their families across Montgomery County to build social-emotional, academic, workforce and life skills, provide mental-health and case-management supports, and connect young people to education and employment opportunities. Their programs (school-based and community-based wellness centers, youth opportunity centers, and workforce development) aim to help youth succeed in school, prepare for work, and strengthen family stability.

AIDS Action Baltimore, Inc. (AAB) is dedicated to providing patient services for all people in Baltimore City regardless of race, religion, age, disabilities, sexual orientation, gender or non-confirming gender, including emergency financial assistance, HIV education, testing assistance, navigation support for healthcare linkage, retention in care, medication adherence and access support, HIV prevention services, including outreach efforts and pre-exposure prophylaxis (PrEP) medication access.

Hudson Pride Connections Center (HPCC) is a home and voice for the diverse LGBTQ community and our allies that advocates for our physical, mental, social and political well-being. We create safe and vibrant spaces to gather and celebrate our lives. HPCC is a 501(c)(3) non-profit, non-governmental, community organization situated in the Journal Square neighborhood in Jersey City, one of the most vibrant and diverse cities in this country and home to the largest LGBTQ community in New Jersey. Hudson Pride was established in 1993 to serve as an advocate for both the LGBTQ and HIV/AIDS communities in Hudson County and more than 20 years later, we are still delivering a wide array of services, programs and events to meet their on-going needs. Our programs and services include LGBTQ support groups for youth, young adults, middle adults and seniors, specialized groups for HIV+ gay men, transgender individuals and women of color, community training and education focused on helping provider agencies to deliver LGBT and HIV competent services, LGBTQ health education, HIV outreach services, social service linkages and advocacy.

ASD currently operates four Special Care Facilities licensed by the Texas Department of Health: The Ewing Center, Revlon Apartments, Hillcrest House, and Spencer Gardens. ASD serves an average of 175 men, women and children in 152 total bedrooms within 125 privately configured units. Seventy-six percent (76%) of the agency's resident population are racial or ethnic minorities, 24 percent were women. One hundred percent (100%) of the people served by ASD are significantly below the federal poverty line and are classified either as low- or very-low income. The agency has provided more than 326,000 person/nights of housing to more than 1,100 individuals and family members living with AIDS. Along with a home and a mutually supportive community environment, residents at each facility are provided with a range of services individually tailored to empower them to cope with the cyclical impact HIV/AIDS has on people who are living with a disease that often compromises their complete independence.

The mission of the Southern Arizona AIDS Foundation (SAAF) is to cultivate a healthy & stigma-free society through transformative action. In 1997, three AIDS service organizations in Tucson (Tucson AIDS Project, PACT for Life, and Shanti Foundation) merged to form SAAF. In 2014, SAAF took over several initiatives from Wingspan, formerly Southern Arizona's LGBTQ community center. In 2015, SAAF celebrated 30 years of providing services and continues to be the only community-based organization in southern Arizona providing case management and ancillary support services for people living with HIV/AIDS and their families; culturally appropriate prevention and education programs to reduce the rate of infection; & LGBTQ community outreach & engagement.

Buddies of NJ, Inc. (NJB), 501(c)(3) is Bergen and Passaic County's Premier AIDS SERVICE ORGANIZATION. Your support makes OUR MISSION possible... to provide: Service, Education, Resources, and Care to People Living With and affected by HIV/AIDS. Buddies provides NJHS Licensed Substance Abuse Counseling & Treatment Services; Free Rapid HIV Testing & Counseling; PrEP Counseling; Case Management; Medical Case Management; Housing & Housing Assistance; Dental Services; Counseling Services; Young Men's Support Spot; Peer Support Groups; Medical Transportation; and a Food Bank. NJB is supported with federal grants from the Ryan White Care Act, HOPWA (Housing Opportunities for People With AIDS), and the New Jersey Department of Health. Additional funding is also provided through private foundations, fundraising efforts, and generous contributions from the community at large. Young 1ove Foundation identifies, adapts and scales up health and education programs, by young people for young people. ​

GAP-A worked on educational activities in global health issues including HIV/AIDS, Avian Flu, Ebola Virus Disease, and Zika.

Through education, prevention, research, care and treatment, and support service, Care Resource improves upon the health and overall quality of life of their diverse South Florida communities in need.
