Day by Day Adult Care provides an adult day program serving older adults and adults with disabilities or chronic illnesses. It offers nursing, social services, therapeutic activities, and supervised daily support designed to keep participants engaged, active, and as independent and healthy as possible.

Metro Meals On Wheels supports the network of neighborhood Meals on Wheels programs across the Minneapolis–St. Paul area to ensure homebound seniors and people with disabilities get nutritious, culturally appropriate meals plus regular friendly contact. The organization focuses on helping recipients remain independent at home by providing meal delivery, enrollment support, volunteer recruitment, and community outreach.

The mission of Meals on Wheels Delaware is to ensure, through the provision of financial and human resources, that every eligible senior in the State of Delaware receives a hot, nutritious meal in their home. • We will continuously engage in fundraising activities and programs, and will strive to improve the effectiveness and efficiency of our fundraising activities.• We will actively engage ourselves in communities throughout Delaware to increase public awareness and promote volunteer support of Meals on Wheels programs.

VNA Hospice & Palliative Care, part of VNA Care, improves the quality of life for individuals and their loved ones facing a life-limiting illness and loss. Our team provides physical, emotional, social, practical, and spiritual support tailored to each person’s needs and goals. Care is provided wherever a patient calls home, including private residences, assisted living and skilled nursing facilities. VNA Hospice & Palliative Care also offers three home-like hospice residences (Elizabeth Evarts de Rham Hospice Home, Cambridge; Rose Monahan Hospice Home, Worcester; and Stanley R. Tippett Hospice Home, Needham) for times when remaining at home is not possible. Our staff is dedicated to our mission to provide the right care with kindness and compassion every day for every person we serve.

Our mission is to empower and advocate for those impacted by aging and disability. We offer information and supportive services that enhance health, well-being and independence.

THE EAST SIDE SOUP KITCHEN WILL PROVIDE A NUTRITIOUS LUNCHTIME MEAL MONDAY THROUGH FRIDAY TO ALL WHO COME TO THE DOOR WITH DIGNITY AND RESPECT FOR EACH PERSON. THE EAST SIDE SOUP KITCHEN WILL ALSO PROVIDE AN AFTER-SCHOOL MEAL TO CHILDREN AT ALL RECREATION CENTERS REQUESTING SERVICE IN WRITING.

Homesteads for Hope is a unique Community Farm experience for transitioning young adults with and without disabilities providing opportunities to learn, work, live and grow in the Greater Rochester Region. Our membership, programs, services and inclusive events are hosted on a 55 acre farm located on the banks of the historic Erie Canal creating a venue of dignity, purpose and sustainable agri-tourism. Our Mission at Homesteads for Hope is to promote sustainable lifestyles and futures by creating a collaborative community to serve all individuals without distinction.Our Vision at Homesteads for Hope is to develop an intentional agricultural-style community that offers an inclusive environment to learn, work, live and grow.

Vincentian Home (part of the Vincentian Collaborative System) provides a continuum of compassionate care for older adults and vulnerable community members in the Pittsburgh area. Guided by Catholic Vincentian values, it offers services ranging from independent living and childcare to memory care, short‑term rehabilitation and long‑term skilled nursing while preserving residents’ dignity.

Visiting Homemaker Service of Hudson County, Inc. (VHS) has been helping Hudson County families with their home care needs since 1959. Due to the ever increasing demand to stop and prevent child abuse, VHS is going Beyond Home Care by focusing on the expansion of the Family Services Department. VHS offers a comprehensive set of home care services and we are proud to say our professional staff has an average of fifteen years’ experience with our agency.We strive to improve the human condition by making sure that every life we touch maintains the dignity, belonging, and independence we all deserve.
