Simon House Recovery Centre

Simon House is one of the highest performing addiction treatment programs in North America. Phase One consists of a seven week program where intensive individual and group sessions are delivered in concert with a personally designed recovery program. Each plan will foster empowerment and an abstinence life style. Upon completion of Phase One, our clients have the option to access our Phase Two and Phase Three which provides ongoing support into transitional housing and independent living. Simon House is the recipient of the 2007 William H Donner Award for Excellence in the Delivery of Social Services, the 2007 Peter F. Drucker Award for Non Profit Management and the 2005,2007 and 2008 winner of the Donner Canadian Foundation Award for Excellence in the Treatment and Prevention if Substance Abuse. Simon House is an independent agency, which does not recieve government funding for its program.

Watari Counselling & Support Services Society

Watari, meaning a "small bridge" or "transition" in Japanese, was registered as a charitable organization in 1986. We serve high-risk youth, their families and their communities in the Greater Vancouver area with a focus on East Vancouver. We run a non-residential drug and alcohol treatment program for 13-24 year olds, formal and group counselling for youth and adults, substance misuse education and prevention curriculum for children in schools and community groups, and a range of related community education, training and development programs. We also work with folks going through the Downtown Community Courts, provide youth outreach on the Downtown Eastside as well as intensive case management for vulnerable youth. We work with over 7000 individuals a year across our many programs and focus on a person's ability to make healthier choices when they have more information.

THE CATHOLIC CENTRE FOR IMMIGRANTS OTTAWA

• Since 1954 (54 years) CIC has welcomed to Ottawa more than 80,000 newcomers of many religions and backgrounds from every part of the world. • CIC operates from 2 locations in Ottawa with 50 staff, a budget of almost $3M, all supported by over 600 volunteers. CIC provides: • temporary shelter and housing assistance to incoming refugees and homeless immigrants, • orientation, settlement and integration services to newcomers, • Canadian friends for newcomers, • help in organizing sponsorships, • actions to prevent families from being evicted from their homes, • assistance to foreign trained professionals, • assistance to the Ottawa Police Service to improve relationships between the Service and immigrant communities. • CIC established the Catholic Immigration Centre Foundation (CICF) in 1997 as a fundraising organization in support of CIC. • The Catholic Immigration Centre is the agency of choice for refugees in Ottawa

Rural Response for Healthy Children

Our agency is dedicated to solving the vast problem of child abuse. RRHC is a community driven and community funded agency that attempts to prevent child abuse through education and community awareness. Rural Response for Healthy Children has been in operation since 1987. We are well known for the successes we have had in the following community programs: We have successfully built community partnerships with agencies such as; the Children's Aid Society (developed disclosure procedures for child abuse) partnered in the Booster Club for Kids, Huron County Health Unit, and Ontario Early Years programs, to name a few. Rural Response for Healthy Children is a well-respected agency in the county and we are known for our strong volunteer programs and ability to coordinate partnerships. We were awarded by the National Projects Fund, the opportunity to develop and disseminate a "volunteer tool kit" for nationwide distribution.

RENFREW AND AREA SENIORS' HOME SUPPORT INC

Renfrew and Area Seniors' Home Support is a non-profit, charitable organization that has been providing volunteer-based support services for seniors and adults with disabilities since 1987. Practical services provided include 1. transportation to out-of-town medical services and to bring clients who live in the outlying townships surrounding Renfrew into Renfrew for medical appointments and other essential services, 2. home maintenance (indoor housekeeping, outdoor groundswork), 3. frozen meals (a selection of over 25 entries), 4. grocery delivery service within Renfrew, 5. client intervention and assistance help with forms, information on various community services and general advocacy assistance, 6.friendly visiting (provide weekly visits to prevent isolation/loneliness) and 7. telephone security checks (regular phone calls to ensure all is well).

Maison d'Hérelle

Maison d'Herelle (St-Hubert St.), created in 1990 is a community hospice that can receive up to 12 people living with HID/AIDS with an important loss of physical or psychological autonomy (palliative care, transitional, convalescence). The Studios (Ste-Catherine St.), was created in January 2007, to provide 15 rented apartments in order to adapt to the changing needs of people living with HIV/AIDS (insecurity, risk of living on the street, health difficulties, dependencies). The Satellite created in February 2006, in Côte-des-Neiges, is a 6 bedroom community appartment, for people living with HIV/QAIDS with the after-effects left by the illness, hence preventing them from living in institutional long term care hospices. The community support and home reinstatement prepare people with HIV/AIDS to lead an active life and to benefit from follow up and support services by our team.

LIBERTY LANE INC

Liberty Lane Inc. is a charitable organization, offering a Second Stage Housing Program and a Family Violence Prevention Outreach Program to women and children who have experienced family violence. Second Stage Housing is a one year program - women have their own apartments with affordable rent in our building and work on goals, healing and moving on. Outreach services are out in the community - providing self-development and self-help groups. Supporting women who are in or who are leaving an abusive relationship by linking them to resources, accompanying them to appointments and safety planning. Children who have been exposed to family violence in their home can attend our "Moving Forward" program that teaches safety planning, emotions, boundary setting and healthy behaviours. Mothers attend a concurrent group to find out what their children are learning in order to support that at home.

Through training workshops, forums and national conferences we provide youth with the skills and knowledge to make a real difference in our communities. Edmonton staff work with at-risk youth, helping them move out of harm's way. We believe that nothing stops youth violence like education or employment. Island Prostate Centre

Moving into our 14th year of operation, The Prostate Centre is here to help men and their families through the decision-making and healing phases of prostate cancer. From prevention and screening, to treatment and recovery, we provide support every step of the way. Prostate cancer is the leading type of cancer in men: one in seven Canadian men will be diagnosed with prostate cancer in their lifetimes. In 2013, 811 men on Vancouver Island were diagnosed with prostate cancer, and 109 died from it. To support this demographic of men, our current programs include: - Full library of books, DVDs, magazines and informational pamphlets on prostate cancer. - Nurse Counseling Program: providing clear, comprehensive, unbiased medical information. - Prostate Support Association: monthly support group meeting, providing peer to peer support for men who are living with prostate cancer.

FORT YORK FOOD BANK (FYFB)

The Fort York Food Bank was founded in February 1998. We originally focused on providing emergency food supplies, but have witnessed a substantial change in the needs of our clients. We have evolved into a multi-service agency providing an integrated set of services briefly described as follows. • Our Food Program provides a three-day supply of groceries, consistent with the Canada Food Guide, to low income individuals and families within our community. • Our Information & Referral program prevents homelessness and eliminates hunger by assisting our clients resolve the underlying issues affecting them. • Our Community Drop-In Centre delivers a range of personal services in a safe environment including meals, internet/computer access along with clothing and books. • Our Community Vitality program helps engage our client base with the broader community.

People For a Healthy Community on Gabriola Society

PHC provides direct services & support by islanders to islanders. The programs and services we offer are aimed at mitigating the impact of isolation, poverty, unemployment, mental illness and addictions. We believe that all of us are needed to help fulfill our aim of an inclusive, strong, prosperous community that is responsive, caring and sustainable. By building local capacity and connections we strengthen the social networks that make Gabriola a safe and healthy place to live. NANAIMO BRAIN INJURY SOCIETY

The Nanaimo Brain Injury Society is a registered non profit society and registered charitable organization. We are a progressive society providing support, education and rehabilitation to people with brain injury since 1988. Creative, energetic and dedicated, our members, volunteers and staff give the society a firm foundation on which we build firm relationships with clients, family members and our wider community. We believe that individuals with disabilities reflect the normal diversity of the human community. Our experience teaches us that most people wish to continue being part of their community following brain injury, and we take seriously the idea of building independence with minimum professional supports. Through our programs and services, people with acquired brain injuries can go on to enjoy fuller inclusion and participation in the community, which includes becoming local advocates for understanding brain injury and its prevention.
